why do good pages still miss the box
Almost always because of the headings. A page will cover a question thoroughly under a heading like Coverage Options or Our Process, which describes the topic and matches nothing anyone types. Google is matching a question against your heading, so a page can be the best answer on the internet and still be structurally unable to be chosen.
The fix is usually not new content. It is rewriting the headings you already have into the questions the content already answers, then tightening the first sentence under each one. That is a small edit with a real ceiling on it, and it is the cheapest work in search.
